When should we seek consultation for fertility?

Anytime, some do it before they start planning but majority would seek a consultation by 6 to 12 months of trying for pregnancy.

What happens during the first consultation?

Medical / gynecological history is obtained and basic clinical examination is done. A pelvic ultrasound is performed. The lady needs a blood test for hormonal profile while the man requires a sperm analysis. The consultation and ultrasound would cost around RM200-250.

Are there Fertility Screening Packages available?

Yes, there are. A simple fertility screening package, that consists of blood test, sperm analysis, consultation and ultrasound for RM550 and a comprehensive package for RM1199. Please call the Fertility Careline (0193381681) for further details.

Oral medications are usually the first-line treatment which increases the likelihood of conception and pregnancy. It can be to stimulate ovulation in women, specific supplementation for men/women or occasionally medication to treat erectile dysfunction in men.
Both partners will be counseled on lifestyle modifications to increase fertility chances.

What are other treatment options?

This would depend upon the specific fertility problem faced by the couple.
The options are the use of more potent medications that come in the form of injections for ovarian hyperstimulation, intrauterine insemination of washed husband’s sperm (IUI) and finally in-vitro fertilization (IVF). IVF process involves ovarian stimulation with potent hormonal injections, egg collection under anaesthesia, fertilizing sperm with an egg in a laboratory. Once the fertilization process is complete one embryo is transferred into the uterus, while the rest of embryos are frozen for future use.

How long will the treatment take?

IUI involves 1-2 ultrasound clinic visits and half day for the IUI procedure.
IVF will require 14 to 17 days, comprising of 3 clinic visits, egg collection (daycare procedure-one day) and embryo transfer (half day)

What is the cost of treatment?

Oral medications can be obtained in most pharmacies. A repeat ultrasound will be needed for monitoring of egg growth which will cost around RM120 – 150.

What is intracytoplasmic sperm injection?

It is a procedure done in the embryology laboratory where a single spermatozoon is injected into the oocyte cytoplasm.

How do primary ovarian insufficiency and ovulation disorders affect female fertility?

Primary ovarian insufficiency means a lower number of eggs in the ovary based on age, and therefore fertility rates drop significantly as the person gets older. She may possibly reach menopause earlier. Ovulation disorders are situations where ovulation (the release of the egg) does not occur or happens at a later date, therefore reducing the chance of pregnancy.

What parameters are used to determine sperm quality?

The common parameters used are the total count, motility, and shape of the sperm.

What causes contribute to abnormal sperm production?

Many factors contribute. Among these factors are lifestyle, smoking, obesity, sexually transmitted diseases, testicular-related diseases/problems, certain medical conditions, or medications.

Can mobile phones affect sperm health?

Yes, it is a possibility. Therefore, it’s best to keep the phone in the shirt pocket.

IVF OVERVIEW

Depends on maternal age, sperm & egg quality, gynecological issues

Ovarian stimulation

To maximise number of egg growth by giving injections. Injections need to be maintained in cold chain storage. Normally given for 10 days. Trans-vaginal ultrasound done to monitor egg growth – minimum 3 ultrasound visits.

Egg collection

Done under general anesthesia to avoid pain. It is a daycare procedure at the Sunway Fertility Centre. Fasting for 6 hours required. Husband gives the semen sample on the same day after egg collection.

Embryology related

Embryologist will inject the eggs with the sperm on the same day. The next day onwards they will update on the progress of embryo growth.

PGT-A

Genetic testing are done on day 5 embryos prior to freezing. It is to screen for genetic abnormality example Down Syndrome, Edwards/Patau Syndrome, etc

Embryo transfer

Fresh transfer done on day 3 or day 5 after egg collection. Frozen cycle (after subsequent menses) is performed using frozen embryos and it mainly involves oral/vaginal medication. Transfer is done on day 3 or day 5 (blastocyst). Normally one embryo is transferred, occasionally two.

Balance embryos

Blastocyst are frozen for future use.
